부모님이 이용할 요양원이나 주간보호센터를 알아볼 때 보호자들이 가장 중요하게 생각하는 것은 결국 ‘돌봄의 질’입니다.
시설이 깨끗한지, 프로그램이 얼마나 다양한지도 중요하지만 실제로 필요한 인력이 배치되어 있는지, 어르신의 상태에 맞는 서비스가 제공되는지, 부족한 부분을 꾸준히 점검하고 개선하는지가 더욱 중요합니다.
최근 국민건강보험공단은 장기요양기관이 스스로 서비스 기준을 확인할 수 있도록 ‘장기요양 서비스 모니터링 자가진단’을 운영하고 있습니다.
🌿 장기요양 서비스 모니터링 자가진단이란?
2026년 8월 국민건강보험공단 발표에 따르면 장기요양급여비용 가운데 일정한 가산을 적용받는 기관은 업무포털을 통해 주요 기준을 스스로 점검할 수 있습니다.
대표적인 확인 내용에는
인력 추가배치
맞춤형서비스 제공
방문요양 사회복지사 등 배치 가산
등이 포함됩니다.
가산 유형에 따라 약 15~17개 항목을 확인하게 되며 기관은 매월 한 차례 자가진단을 실시할 수 있습니다.
다만 이 제도를 모든 요양원과 주간보호센터를 대상으로 하는 새로운 ‘기관평가’라고 이해해서는 안 됩니다.
국민건강보험공단의 장기요양기관 정기평가와는 별도로, 가산기준을 적용받는 기관이 스스로 기준 준수 여부를 살펴보고 부족한 부분을 개선하기 위한 자율점검 제도에 가깝습니다.
2026년 6월 기준 자가진단 대상 기관 2만3,795곳 가운데 1만8,075곳이 참여해 약 76%의 참여율을 기록한 것으로 발표됐습니다.
이처럼 장기요양서비스도 이제 단순히 기준을 한 번 갖추는 데 그치지 않고 지속적으로 확인하고 개선하는 과정이 중요해지고 있습니다.
Why Self-Monitoring Matters in Long-Term Care
Choosing a nursing home or an adult day care center for an aging parent is rarely an easy decision.
Families often begin by looking at the building, location, meals, activities, or outdoor environment. These factors certainly matter, but the quality of long-term care depends on something deeper: whether the organization consistently follows proper care standards.
In Korea, the National Health Insurance Service has introduced a self-check system that allows eligible long-term care providers to review whether they are meeting specific requirements associated with additional reimbursement.
The system is designed to encourage providers to identify potential problems before they become larger issues.
What Does the Self-Check System Review?
Depending on the type of additional reimbursement a provider receives, approximately 15 to 17 items may be reviewed.
These can include areas such as:
Staffing above the minimum requirement
Provision of individualized or specialized services
Placement of social workers and other qualified personnel in home-care services
Compliance with operational requirements related to additional reimbursement
Participating organizations can complete the self-check through the long-term care provider portal.
The process is generally conducted once a month.
This kind of routine monitoring can help an organization recognize whether its actual operations remain consistent with the standards under which additional reimbursement is being received.
Self-Check Does Not Mean a Full Facility Evaluation
There is an important distinction for families to understand.
This self-monitoring program is not the same as the official periodic evaluation of long-term care institutions conducted by the National Health Insurance Service.
It also does not mean that every nursing home or adult day care center in Korea receives a new monthly government rating.
Instead, the program focuses on eligible providers receiving certain additional payments and allows them to examine whether they continue to meet the relevant requirements.
Therefore, a self-check result should not be interpreted as a complete judgment of whether a facility is “good” or “bad.”
The broader quality of care must be considered through many different factors.
Why Regular Monitoring Is Important for Older Adults
Older adults receiving long-term care often experience changes in their physical or cognitive condition.
Mobility may decline.
Eating and swallowing ability can change.
Fall risk can increase.
Skin condition may deteriorate.
Medication needs may change.
A person who was able to walk independently several months ago may suddenly require assistance.
This is why long-term care cannot rely only on a care plan that was created once.
Care needs should be reviewed continuously.

What Families Should Look for When Choosing a Care Facility
A beautiful building alone does not guarantee good care.
When visiting a nursing home or adult day care center, families may want to observe how staff members interact with older adults.
Do staff speak respectfully?
Are residents assisted safely when walking or transferring?
Are meals and hydration carefully managed?
Are changes in health condition noticed and communicated?
Are activities appropriate for the person's physical and cognitive abilities?
How does the organization respond when a problem is identified?
Families can also ask how the facility manages fall prevention, pressure injury prevention, medication support, nutrition, hygiene, emergency situations, and communication with guardians.
There is no single indicator that determines the quality of a long-term care facility.
Instead, good care usually comes from many small practices being performed consistently.
Continuous Improvement Is Part of Good Care
Long-term care is built through everyday routines.
Helping someone get out of bed safely.
Supporting a person during meals.
Watching for signs of dehydration.
Preventing falls.
Encouraging appropriate physical and cognitive activity.
Listening when an older adult expresses discomfort.
Reporting meaningful changes in health.
None of these actions may appear dramatic, but together they form the foundation of safe and respectful care.
For this reason, systems that encourage providers to review their own practices can be valuable.
The goal should not simply be to complete a checklist.
The real purpose is to identify what can be improved and then make meaningful changes in daily care.
