우리나라 노인 돌봄의 방향

 🌿아파도 살던 집을 떠나기 싫어하시는 부모님!

장기요양이 필요해진 뒤에도 어르신들은 어디에서 생활하기를 원할까요?


보건복지부가 2026년 8월 발표한 「2025년 장기요양실태조사」는 우리나라 노인돌봄의 방향을 생각하게 하는 중요한 결과를 보여줬습니다.

재가 장기요양 수급자 10명 중 약 7명은 건강이 나빠지더라도 현재 살고 있는 집에서 계속 생활하기를 희망하는 것으로 조사됐습니다.

오랫동안 살아온 집은 단순한 주거공간이 아닙니다.

익숙한 방과 가구, 이웃, 동네 풍경과 생활습관까지 한 사람의 삶과 연결되어 있습니다. 그래서 가능한 한 살던 곳에서 노후를 보내고 싶다는 마음은 자연스럽습니다.

하지만 여기에는 중요한 문제가 있습니다.

“집에서 살고 싶다”는 희망과 “집에서 안전하게 생활할 수 있다”는 것은 항상 같지 않습니다.

노화나 질환으로 신체기능과 인지기능이 저하되면 식사, 목욕, 이동, 배설, 복약관리 등에 도움이 필요해질 수 있습니다.

낙상이 반복되거나 치매로 인한 배회와 야간 행동 변화가 나타나고, 연하곤란이나 욕창 위험 등으로 지속적인 관찰이 필요한 경우도 있습니다.

따라서 부모님의 희망을 존중하면서 현재 건강상태와 가족의 돌봄 능력을 함께 살펴야 합니다.

장기요양보험에서는 방문요양, 방문목욕, 방문간호, 주·야간보호, 단기보호, 복지용구 등 가정생활을 지원하는 다양한 재가급여를 운영하고 있습니다.

그러나 장기요양등급과 개인별 이용계획, 서비스 종류 등에 따라 이용 가능 범위와 비용이 달라질 수 있으므로 개별 확인이 필요합니다.

가족이 낮 동안 부모님을 돌보기 어려운 경우에는 주간보호센터를 검토할 수 있으며, 24시간 지속적인 도움이 필요한 단계가 되면 요양원과 같은 시설 돌봄도 함께 고려할 수 있습니다.

중요한 것은 어느 한 가지 돌봄 방식이 모든 어르신에게 가장 좋다고 단정하지 않는 것입니다.

부모님이 비교적 건강하실 때부터 앞으로 어떤 생활을 원하는지 가족과 이야기를 나눠보는 것이 좋은 돌봄 준비의 시작입니다.


🌿 Aging in Place: What Korea’s Latest Long-Term Care Survey Tells Families

Where would you want to live if you became frail and needed help with everyday activities?

For many older adults, the answer is simple: home.

According to the 2025 Long-Term Care Survey released by Korea’s Ministry of Health and Welfare in August 2026, roughly seven out of ten long-term care recipients living in the community expressed a desire to remain in their current homes even if their health deteriorated.

This preference reflects a concept widely known as “aging in place” — the desire to grow older while remaining in a familiar home and community for as long as possible.

For families caring for aging parents, however, respecting that preference requires more than simply keeping a loved one at home.

It requires planning.

🌱 Why Home Matters So Much

A home is more than a building.

For someone who has lived in the same place for many years, everyday surroundings can carry deep familiarity: the bedroom, the kitchen, the neighborhood, nearby shops, familiar faces and established routines.

These surroundings can provide continuity at a time when physical or cognitive abilities may be changing.

This is one reason many older adults prefer community-based care whenever it remains safe and practical.

But there is an important distinction.

Wanting to stay at home does not necessarily mean that living at home without additional support is safe.

🌿 When Everyday Care Needs Begin to Change

Aging does not affect everyone in the same way.

Some people remain independent well into advanced age, while others gradually need assistance because of illness, frailty, cognitive impairment or reduced mobility.

Families may first notice relatively small changes.

An older parent may begin having difficulty preparing meals, taking medications correctly or keeping the home organized.

Later, more significant assistance may be needed with bathing, dressing, toileting, walking or transferring from a bed to a chair.

Repeated falls deserve particular attention.

So do wandering, major changes in nighttime behavior, difficulty swallowing, significant weight loss and an increasing need for assistance with most daily activities.

These changes do not automatically mean that residential care is necessary.

They do mean that the person’s care needs should be reassessed.

🌿 Supporting Aging in Place

Korea’s Long-Term Care Insurance system includes a range of home- and community-based services.

Depending on eligibility and an individual's care plan, these may include home-visit care, bathing assistance, nursing services, day and night care, short-term care and assistive equipment.

These services can help some older adults remain at home longer while reducing part of the caregiving burden placed on families.

However, eligibility, available benefits, service limits and out-of-pocket costs can differ according to the individual’s long-term care grade and circumstances.

Families should therefore confirm the services that actually apply to their situation rather than assuming that every benefit is automatically available.

🌱 The Role of Adult Day Care

Adult day care can provide another option for families who want to support an older person’s life at home.

An older adult can spend part of the day at a care center and return home afterward.

Depending on the center and the individual care plan, the day may include meals, assistance with everyday activities and appropriate physical, cognitive or social activities.

This arrangement may be especially helpful when family caregivers are working during the day.

🌿 When Residential Care May Need to Be Considered

There may eventually come a time when living at home becomes increasingly difficult despite community support.

For example, an older adult may require assistance throughout the day and night, experience frequent falls or need extensive help with eating, mobility, toileting and other activities of daily living.

Cognitive changes may also make continuous supervision necessary.

In these circumstances, families may need to consider whether a long-term care facility can provide a safer and more sustainable care environment.

Choosing residential care should not automatically be viewed as abandoning the goal of aging in place.

Care needs change.

The appropriate environment may therefore change as well.

🌿 Talk About Care Before a Crisis Happens

One of the most useful things families can do is discuss future care while their parents are still able to clearly express their wishes.

Ask questions such as:

“Would you like to stay at home as long as possible?”

“What kind of help would you be comfortable receiving?”

“How would you feel about attending a day care center?”

“If you eventually needed help around the clock, what would matter most to you?”

These conversations can make later decisions less confusing and help families understand what their loved one truly values.

The latest long-term care survey reminds us of something important.

Older adults want more than medical treatment and physical assistance.

They want continuity, familiarity, dignity and as much choice as possible in deciding how they live.

The goal of good long-term care should therefore not be to force every older person into the same model.

Home care, community services, adult day care and residential care can all play different roles as a person’s health and care needs change.

The best preparation begins before a crisis.

It begins by listening.
