부모님의 건강을 살필 때는 질병명만 확인하지 말고 실제 생활의 변화를 관찰해야 합니다.
최근 걸음이 느려졌는지, 의자에서 일어나기 어려워하는지, 식사량이나 체중이 줄었는지, 약 복용을 자주 잊는지, 같은 질문을 반복하는지 살펴보세요. 목욕, 옷 갈아입기, 식사 준비, 화장실 이용, 외출과 같은 일상생활에서 도움이 필요한 부분도 기록해 두는 것이 좋습니다.
갑작스러운 의식 변화, 말이 어눌해짐, 한쪽 팔다리의 힘 빠짐, 심한 호흡곤란, 가슴 통증, 반복되는 구토, 심각한 탈수 증상이 나타나면 돌봄 상담보다 먼저 의료기관의 신속한 평가가 필요합니다.
부모님의 의사를 먼저 확인하세요
돌봄 계획의 중심은 부모님이어야 합니다. 건강이 더 나빠졌을 때 누구에게 도움을 받고 싶은지, 집에서 생활하기 위해 어떤 지원이 필요한지, 주간보호센터나 요양원에 대해 어떻게 생각하는지 차분히 대화해 보세요.
부모님이 비교적 건강하실 때 의견을 나누면 위급한 상황에서 가족들이 서로 다른 판단을 하며 갈등하는 일을 줄일 수 있습니다.
가족의 돌봄 능력을 현실적으로 점검하세요
가족의 사랑만으로 모든 돌봄을 감당하기는 어렵습니다. 주 보호자가 누구인지, 낮과 밤에 돌봄이 가능한지, 병원 동행과 약 관리는 누가 담당할지, 비용은 어떻게 분담할지 구체적으로 정해야 합니다.

장기요양보험과 지역 돌봄서비스를 알아두세요
노인장기요양보험은 나이만으로 자동 이용하는 제도가 아닙니다. 국민건강보험공단의 인정조사와 의사소견서 제출, 등급판정 등의 절차를 거쳐 수급 여부가 결정됩니다. 실제 이용 가능한 서비스와 본인부담금은 장기요양등급, 급여 종류, 감경 대상 여부 등에 따라 달라질 수 있습니다.
주간보호센터는 어르신이 낮 동안 센터에서 식사, 돌봄, 인지·신체 프로그램 등을 이용하고 저녁에 가정으로 돌아오는 형태입니다. 요양원은 지속적인 관찰과 생활 지원이 필요한 어르신이 입소하여 생활하는 장기요양기관입니다.

Five Care Preparations Families Should Make Before a Parent’s Health Changes
As parents grow older, families often focus on diagnosed illnesses. However, changes in everyday function may provide equally important information about an older adult’s health and care needs.
A parent may begin walking more slowly, eating less, losing weight, forgetting medication, repeating questions, or needing more help with bathing and dressing. Each change may appear minor on its own, but a pattern of decline deserves attention.
Preparing for care does not mean forcing a parent to enter a nursing home. It means understanding their wishes, monitoring their abilities, improving safety, and learning which services may be available before an urgent situation occurs.
Observe Changes in Daily Life
Families should pay attention to what an older parent can actually do during an ordinary day.
Can they rise from a chair without assistance? Are they able to walk safely to the bathroom? Can they prepare food, take medication correctly, use the telephone, and manage transportation? Have they experienced falls, reduced appetite, confusion, or withdrawal from social activities?
The World Health Organization identifies cognitive decline, limited mobility, undernutrition, impaired vision or hearing, depressive symptoms, urinary incontinence, and falls as important areas to assess in older adults. These changes do not automatically mean that a person has a particular disease, but they may indicate that a medical or functional assessment is needed.
Families can keep a simple notebook recording falls, appetite, sleep, medication mistakes, unusual behavior, and the amount of assistance required. A record of gradual changes can be useful when speaking with a doctor or care professional.
Sudden symptoms require a different response. New confusion, difficulty speaking, weakness on one side of the body, severe chest pain, breathing difficulty, repeated vomiting, loss of consciousness, or signs of severe dehydration may require urgent medical attention.
Discuss the Parent’s Wishes Early
Care decisions should be made with the older person, not merely about them.
Ask where your parent would prefer to live if more assistance becomes necessary. Discuss which daily activities matter most to them, who they trust to help with healthcare decisions, and how they feel about home-based care, a day-care center, or residential care.
These conversations can feel uncomfortable, but they are usually easier when everyone is calm and the parent can express personal preferences clearly. Waiting until a fall, hospitalization, or sudden cognitive decline may leave the family with little time to compare options.
The goal is not to obtain a final answer in one conversation. Preferences may change as health conditions and family circumstances change. The discussion should therefore be revisited periodically.
Evaluate the Family’s Real Capacity to Provide Care
Family care is meaningful, but it also requires time, physical strength, emotional energy, and money.
A realistic plan should answer several questions. Who will be the primary caregiver? Who can accompany the parent to medical appointments? Who will manage medication and finances? Is anyone available at night? What happens if the main caregiver becomes sick or needs rest?
It is important to be honest. A family member who works full-time may not be able to provide constant supervision. An older spouse may not be physically able to assist with transfers, bathing, or toileting. Trying to manage beyond the family’s capacity may increase the risk of injury, medication errors, exhaustion, and conflict.
Seeking professional support is not a failure of family responsibility. Respite services, community programs, home-based services, and day-care services may help both the older person and the caregiver maintain a safer routine.
Learn About Long-Term Care Options Before a Crisis
In Korea, eligibility for Long-Term Care Insurance is not determined by age alone. An application is followed by an assessment of the individual’s physical function, cognition, behavioral changes, nursing needs, and rehabilitation-related condition. A medical opinion and formal grade determination may also be required.
Available benefits, personal expenses, and service conditions can vary according to the person’s long-term care grade, type of benefit, individual circumstances, and applicable reduction rules. Families should therefore confirm current details with the National Health Insurance Service rather than relying only on informal information.
A day-care center generally supports older adults during the daytime and allows them to return home afterward. Depending on the center and the individual care plan, services may include meals, personal assistance, cognitive activities, physical activities, and transportation support.
A nursing home is a residential option for people who need ongoing assistance and supervision. It may be considered when living safely at home has become difficult, nighttime care is repeatedly needed, or the family can no longer provide the required level of support.
Neither option is automatically better. The appropriate choice depends on the older person’s health, functional ability, preferences, home environment, and family support.
Visit and Compare Care Providers
Families should visit more than one provider whenever possible.
A clean and attractive building is important, but it does not tell the whole story. Observe how staff members speak to residents, whether older adults are treated respectfully, how meals and hygiene are managed, and how the provider communicates with families.
Ask how the provider responds to falls, sudden health changes, swallowing difficulties, behavioral symptoms, and hospital visits. Confirm which services are actually available and which may require separate arrangements or additional costs.
A good provider should be willing to explain its care process clearly. Families should feel comfortable asking questions and should not feel pressured to make an immediate decision.
Early Preparation Protects Both Parents and Families
The future of care cannot always be predicted. A parent’s condition may remain stable for years, or it may change after an illness, fall, or hospitalization.
However, families can prepare by observing daily function, discussing preferences, sharing responsibilities, learning about available services, and comparing providers in advance.
Early planning helps the family respond calmly when circumstances change. More importantly, it helps preserve the older person’s dignity, choices, relationships, and familiar way of life for as long as possible.
