파킨슨병은 시간이 지나면서 점차 진행되는 퇴행성 신경계 질환입니다. 말기에는 단순히 손이 떨리는 증상을 넘어 걷기 어려워지고, 음식 삼키기가 힘들어지며, 의사소통과 인지 기능에도 변화가 나타날 수 있습니다.

대표적인 말기 증상은 보행장애, 심한 근육 강직, 연하장애(삼킴 곤란), 의사소통 장애, 인지 기능 저하, 배뇨장애와 기립성 저혈압 등 자율신경 증상입니다.

모든 환자가 같은 과정을 겪는 것은 아니지만, 적절한 치료와 재활, 보호자의 따뜻한 돌봄은 삶의 질을 유지하는 데 큰 도움이 될 수 있습니다.

특히 낙상 예방, 충분한 영양 공급, 안전한 식사 환경, 규칙적인 약물 복용, 의료진과의 지속적인 상담은 매우 중요한 관리 요소입니다.

Parkinson's disease is a progressive neurological disorder that gradually affects movement, balance, speech, and many other body functions. While symptoms vary from person to person, advanced Parkinson's disease often requires continuous support from family members or professional caregivers.

One of the most noticeable changes is severe mobility impairment. Walking becomes extremely difficult, freezing episodes become more frequent, and many patients rely on wheelchairs or spend much of the day in bed. Falls become a major safety concern because balance reactions are significantly reduced.

Muscle rigidity also becomes more severe. The body may feel stiff throughout the day, making even simple movements uncomfortable. Without regular position changes and gentle range-of-motion exercises, joint stiffness and pressure injuries may develop.

Another important challenge is swallowing difficulty, known as dysphagia. Food or liquids may enter the airway instead of the esophagus, increasing the risk of aspiration pneumonia. Caregivers should watch for frequent coughing during meals, prolonged eating times, or repeated choking episodes and consult healthcare professionals when these signs appear.

Speech and communication also become increasingly difficult. The voice often becomes very soft, and words may be difficult to understand. Although communication becomes challenging, patients usually continue to understand conversations, making patience and eye contact especially valuable during daily interactions.

Some individuals may also experience cognitive decline as the disease progresses. Memory, attention, decision-making, and problem-solving abilities can gradually decrease. While Parkinson's disease is different from Alzheimer's disease, some patients eventually develop Parkinson's disease dementia.

Non-motor symptoms become equally important during the advanced stage. These may include constipation, urinary problems, sleep disturbances, anxiety, depression, fatigue, and orthostatic hypotension, which causes dizziness when standing up.

Practical Care Tips for Families

Creating a safe home environment is one of the most effective ways to reduce fall risk. Remove loose rugs, improve lighting, install grab bars when appropriate, and keep frequently used items within easy reach.

Nutrition and hydration should receive close attention. Some patients benefit from softer foods or modified meal textures if swallowing becomes difficult. Medical advice should always guide dietary adjustments.

Medication schedules should be followed consistently because timing can significantly influence symptom control. Family members should also monitor for sudden changes in movement, swallowing, or mental status and seek medical evaluation if needed.

Physical therapy, occupational therapy, speech therapy, and long-term care services may provide valuable support depending on each individual's condition.

Parkinson's Disease vs. Alzheimer's Disease

Although both conditions become more common with aging, they affect the brain differently.

Alzheimer's disease usually begins with memory impairment and gradually affects thinking and daily functioning.

Parkinson's disease typically starts with movement-related symptoms such as tremor, slowness of movement, muscle stiffness, and balance problems. Cognitive decline may develop later in some patients, but the disease process differs from Alzheimer's disease.

Understanding these differences helps families recognize symptoms early and seek appropriate medical evaluation.
